The Georgia Academy of Family Physicians is encouraging physicians in the state to complete a 10-minunte survey developed by the Georgia Cancer Control Consortium and Georgia Department of Public Health (DPH) that addresses HPV vaccine stocking and administration and potential barriers associated with HPV screening and counseling.
This survey is confidential. Your participation in this survey is voluntary and greatly appreciated. Survey results will be reported in aggregate form only and will not be linked to you or your practice.
It will take you approximately 10 minutes to complete this survey. The deadline for completing the survey is July 31, 2015.
