Alliant Quality is working in your community to improve health outcomes among people with diabetes.

Nearly one-third of all persons 65 and older have diabetes, the most common cause of blindness, kidney failure and amputations, and a leading cause of heart disease and stroke.

The Everyone with Diabetes Counts program addresses disparities in the prevalence and treatment of diabetes in underserved and rural populations who are affected at higher rates. Alliant Quality is building a sustainable network of diabetes self-management education (DSME) programs to reach Medicare beneficiaries in their communities.

Alliant Quality will increase the health literacy and empowerment of people with diabetes in your community by:

  • expanding access to free DSME programs to people affected by diabetes by collaborating with existing programs and providing programming in areas of need
  • developing a sustainable network of diabetes educators, including Certified Diabetes Educators (CDEs) and peer educators through a Train-the-Trainer Program
  • working with providers to improve quality of care for people with diabetes through free technical assistance to address quality measures
  • increasing the number of accredited diabetes education programs, such as those recognized by the American Diabetes Association (ADA) or the American Association of Diabetes Educators (AADE)
