Cardiac Quality Improvement Initiative

Alliant Quality is seeking all healthcare practitioners, medical practices, patients and families, community organizations, and Million Hearts® stakeholders both state and national, to join efforts to improve and measure progress on the ABCS of cardiovascular health:

Aspirin as appropriate
Blood pressure control
Cholesterol management
Smoking screening and cessation counseling

Heart attacks and strokes are the leading cause in death in our nation! It takes a village of communities to identify the barriers, share best practices, and make a nationwide impact. So, we need your help to do your part in Georgia!

CMS and The Centers for Disease Control and Prevention (CDC) created the Million Hearts® initiative to bring together communities, health systems, non-profit organizations, federal agencies, and private-sector partners from across the country to prevent a million heart attacks and strokes before 2017.

ABFM Maintenance of Certification Part IV

The Cardiac Quality Improvement Initiative is an approved alternate activity for Maintenance of Certification (MOC) Part IV—Performance in Practice that satisfies the ABFM MC-FP requirements. Participants who successfully complete this activity will receive MC-FP credit for 1 Part IV module or 20 Part IV points, as appropriate.

Physicians receive the following free assistance during this 1-year project:

  • Onsite and virtual QI coaching on satisfying Part IV MOC requirements
  • Access to patient and providers tools and resources
  • Learning and sharing of best practices on quality improvement processes and methodologies
